MEMBERSHIP
AND NOMINATION FORM
The
membership form contains membership information and lists the applicable
fees. You must have a Family, Individual or Junior membership
in order to nominate a horse or rider to participate in the end
of year awards program. The second part of the form lists
nominations for equitation riders (Hunter/Jumper and Show Horses
and Ponies), horses (all disciplines), amateur adult (Dressage),
and Jr/YR (Dressage). The age listed for equitation riders
is the age as of December 1 of the current competition year..
Please
read the form carefully and provide all requested information.
Incorrect or incomplete forms may result in a delay activating your
membership.
The
show recognition form is for use by horse show secretaries and horse
show managers to register their shows with the MHSA. Please read
the form carefully and provide all requested information.
Incorrect or incomplete forms may result in a delay recognizing
your show. Shows are not recognized and do not appear on the
web site or MHSA publications until the form has been received with
a check for the appropriate fee.
The
show evaluation form allows exhibitors to provide feedback on MHSA
recognized competitions they attend. These evaluations are
confidential, however, the MHSA may investigate any incident reported.
The forms may be submitted to any board member listed on the MHSA
Board page. Comments and/or criticism should be constructive
since the purpose of this form is to improve the quality of recognized
shows and reward those shows already meeting your expectations.
The
Application for MHSA Medallion Class form is for use by horse show
secretaries and horse show managers to register their MHSA Medallion
classes (Hunt Seat, Saddle Seat, Stock Seat). Fee and
mailing instructions are included on the form. MHSA Medallion
classes which have not been registered and for which no fee has
been received will not count toward the Medallion Finals in the
fall.

PERSON OF THE YEAR NOMINATION FORM
Do
you know an MHSA member who deserves special recognition? Does the
MHSA member that you have in mind demonstrate superior horsemanship,
professionalism, sportsmanship and support of the MHSA? Then print
a copy of the trainer of the year nomination form, fill it out and
send it to Jacqueline Wessel!

SILENT
AUCTION DONATION FORM
The
Missouri Horse Shows Association will be holding a silent auction
during their Annual Meeting & Awards Banquet, Saturday, February
5, 2005. The MHSA is looking forward to a wide variety of generous
and imaginative donations from members, friends and equestrian related
businesses in the state. Proceeds from the auction will be used
for MHSA activities aimed at the promotion of the horse industry
in Missouri. Show your support for the MHSA, advertise your business
and come out for the fun at the 2005 banquet!

RIDER
PROFILES
If
you would like to be a part of the new MHSA website, read on! Submit
your information to web site manager Karen Pautz and soon you and
your horse will be "web personalities!" Important stuff
like signatures need to be mailed to Karen, but you can submit your
questionnaire and your pictures electronically if you would like.
(It would also make Karen's life more fun). All photos and content
will be published subject to approval by the MHSA website beautification
committee.
